Introductory material on Outlook Forms

We're a Server2K/Win2K/Office2K shop & want to use the
Outlook forms capability for two internal applications: A
New User Request form (to set up a new account, e-mail &
other administrative stuff for new employees) and an
Office Supplies Request form (to let remore users order
pens, paper & stuff).

We haven't used this feature yet & I'm looking for
introductory (i.e. elementary) info on this.
