Inventory query driven by form questionaire

Hi all - thanks for your time. Sorry for the newbie post.

I need a form which will ask questions of the user, and result in presenting
a subset of items from a database. As follows:

Database contains available types of ice cream!

field one: I am allergic to nuts (y/n radio button)
I prefer chocolate to vanilla (y/n)
I hate marshmallows (y/n)

in the end, the user will be presented with the matches to his query, based
on the form.

I suspect you will not find such a form since it's pretty specific. :) You
can always create exactly what you want or you may need to look into hiring
someone to create it for you since it sounds like what you want is a form
that's bound to a database but not part of the database?
