is there a cd case template?

I'd like to be able to make my own Jewel cases for cd's that I compile. Are
there templates I can use so that all I have to do is add either text or a
picture or both, print it and then cut to size, fold and insert in a stock
jewel case?

Draw a rectangle with 12.1x12.1 cm for the front cover, on the second slide
draw a rectangle with the size 11.8x13.8 cm and two smaller ones with
11.8x0.65 cm on its left and right. And there you are.
