is there a fax cover sheet linked to outlook contacts?

I would like a cover sheet that will allow me to pick from my contact list
and enter the info into the cover sheet.

There is no menu item for a fax cover sheet in Outlook. Normally one would
create and then save a merge document in Word for that purpose. For further
