Is there a good book I can use to teach myself to design forms in.

I would like to design a form to use in the Business Contact Manager that
will allow me to track writing submissions to journals, etc. The Opportunity
form that comes with the product is product based and not really adaptable to
my purposes. So I am wondering if there's a book out there that I can use to
teach myself how to design my own form.
