You can download a PDF of the form from
http://www.irs.gov/pub/irs-pdf/f1098_09.pdf, but you can't file it; the
first page of the file says:
"This form is provided for informational purposes only. Copy A appears in
red, similar to the official IRS form. Do not file copy A downloaded from
this website. The official printed version of this IRS form is scannable,
but the online version of it, printed from this website, is not. A penalty
of $50 per information return may be imposed for filing forms that cannot be
scanned."
I suspect the same would be true of any mockup of the form that someone
created in Word. Order the real thing from
http://www.irs.gov/businesses/page/0,,id=23108,00.html.
What you can do is paste a copy of the form from the PDF into a Word
document and send it behind the text, then insert borderless text boxes and
position them over the blanks in the form. Fill in the text in the boxes.
Finally, remove the background form and print the document (now containing
only your entries) onto the official paper form.
