Is there a way to make a clear button on custom form?

I have created a customer form based on a outlook message form for keeping
notes. Is there a way to create a button that will remove the information
that has been put into the fields so the form is blank again?

Write code that sets the value of each property to "", 0, or #1/1/4501#
depending on whether it's a string, number, or date field. See

I have similar thing. I created a template with date, name, and comment fill in fields i copied from ather templates. I need a clear button to Clear the fields once i'm done, as to start with a clear template. I'm a begginer at creating templates. So please give me the cut and paste version please. LOL
