Issues Database Template - createing DAP

I'm using the Issues database Template and am trying to create a Data Access
Page that looks and works like the issues form.
When I save as the issues form to a data access page I receive the following
error.
Issue tracker is unable to create the data access page
The file does not exist, or you do not have read access to the file.

I've also tried using the wizard and importing the issues table or open
issues query, but I've had no luck.
Is there a way to combine the contacts table and issues table in one DAP
that would be usable?

Or can the above error be rectified?
