IT Helpdesk Form

Has anyone developed their own Helpdesk form for Outlook?

Something simple for creating problems, editing solutions
etc, assigning problems to other helpdesk staff and even
being able to run simple reports.

I have looked at the Microsoft example of the Helpdesk
form but this is quite old now and I don't feel
sufficiently capable of customizing it.

If anyone has an example they are willing to share I'd
appreciate a look at it.
