Junk E-Mail Folder

We have internal messages ending up in the Junk E-mail Folder and junk email
going to the Inbox. There are no rules configured. We are using only accept
safe senders list, which is populated with our domain name. We are running
Exchange 2003 with the 2003 client. Why is the email ending up in the wrong
folders?
