knowing who entered what

I want to create a database for a department that has
about 4-5 groups. I want to be able to secure the
database, so that when a person enters the password, it
will only show the data for their group. The database
will be used to keep notes on each client in the group
for example:

Department is substance abuse
groups are alchol addiction group, drug abuse group

there can be up to 13 clients in a group and after a
meeting, a counselor will type up notes on each client
that attended the group.

Is there a way to put in a field that will allow the
client to electronically sign after the note has been
entered. (what kind of equipment would we need if
possible)?

Also, If a password is required to use the database, is
there any way of letting me know, who entered what? Can
there be some type of stamp that I could view who did what
entry and when?
