Lessons Learned DB

Has anyone done a lessons learned database before? or know
of any shareware/freeware lessons learned db? I have to
design a lessons learned database to manage lessons
learned across multiple projects/teams and I was wondering
if there was anything out there that I could leverage as a
starting point.
