letter of complaint denied health care

I need to find a template to write a letter to a clinic I have been with for
over 8 years they are denying me continued care with my doctor retiring plus
a big factor of this denial comes from a patient there who has maligned me to
the clinic. This person is a compulsive liar very unstable person. I have to
get back to the clinic asap without the treatments my health will not be good
I can't possibly deal with the pain that will become unbearable for me I deal
enough as is but at least the treatments give me some life. This patient did
tell me they were going to get me kicked out which has happened. The letter I
need to write will go to the head of the clinic and it has to be direct and
perfect. A well formed letter is needed. Any help would be appreciated.

Just say what you said in this post, maybe a bit more succinctly. Templates are
simply that, no content, especially as specific as yours.
