letterhead template

I cannot figure out how to make a template of the office
letterhead.

1. Does it go into a heading or just at the top of the
page?

2. How do I import the graphic from the current letterhead
onto the template? I tried scanning the hardcopy
letterhead but it was fuzzy and took up the entire page
rather than only at the top of the page.

I'd appreciate not only learning how to do this, but what
vocabulary I can use to find this information in the HELP
files.
