Linking a Form to a Query?

I put a post in the query newsgroup but realized that I
more people in here would know about forms.

I have a query that has 70 fields...I want to create a
form that has all 70 fields and a checkbox for each one..

If I check 10 of the fields...the query will give me just
those 10 fields.

I am very new to this and I am not sure how to do this.

Sometimes I need 10 fields, sometimes I need 15.
