Linking form box in word

I am making gift certificates and want to put two of the same certificate on
each page, with the same information. I am using form tex boxes as not all
the information on the certificates will change, just the Name, address, etc.
What I want to do is link the form-textboxes on the top certificate with the
corresponding form feild in the bottom certificate, so I don't need to type
the same information twice. Thank you so much for your help!
