List of Figures Omits Captions in Word

I've got a Word document with a list of tables and list of figures after the
table of contents. The list of tables updates fine, but the list of figures
always omits figure 4 (out of 5). Nothing I do will correct this. I've
tried the following:

- Delete and reinsert the caption (several times)
- Toggled the field codes (they look the same as on the other captions)
- Deleted and reinserted the entire figure
- Added a new caption above or below the faulty one (When I update the list
of figures, it will show the second of the two captions, but not the one
labeled 'Figure 4'. If I then delete 'Figure 4' and update the numbering of
'Figure 5' so that it is the new 'Figure 4', it no longer appears on the list
of figures.)

The figure caption will update the figure number when I add or delete
figures before it, so it seems to be working fine. But whenever I update the
list of figures at the beginning of the document, the list skips from figure
3 to figure 5.

What is wrong and how can I fix it?

If the caption is in a text box, the Table of Figures will not see it.

The caption is not in a text box. To make the matter more stupifying, the
caption used to work fine. I recently revised the document, but didn't touch
the caption or anything near it. I had some trouble getting the new table
and figure headings to show up in their respective lists, but eventually got
them all to work. In the process, however, this caption which was working
fine, suddenly stopped showing up on the list of figures.

I have the same exact problem. So far this is the only way I can fix it:
1.) Open a new document.
2.) Select all and copy the content of the document that has this issue.
3.) Paste the content in the new document.

I don't understand why I have to rewrite the document to fix this.
