Long Heading creating two seperate entries in TOC

In the document, the long heading was broken up with a soft return, so I
thought that was the issue and replaced it with a space. When compiled, it
continues to come into the TOC as two seperate entries w/page numbers.

Any Ideas?

Is this long heading preceded by a page break? If so, the same style
will be applied to the actual page break, which could cause the
heading to be duplicated in the TOC (especially if there is at least
one space preceding the break). You can work around this by removing
the page break and then formatting the heading as "Page break before"
(Format | Paragraph, Line and Page Breaks tab).

Unfortunately, the heading is not preceded by a page break, just another
paragraph with a different style.

What happens if you copy the text (but not the TOC and the final
paragraph mark) into a new document? Will the TOC update correctly
then?
