Something like a template which has 'characteristics' of teachers - which can
be provided to the parents prior to the beginning of the school year, so that
they can request certain 'characteristics' for their child's assigned teacher?

Wow! Why not let the parents run the school? Interview job applicants?
Why not let the parents take over the teaching?
I've never heard such rubbish "they can request certain
'characteristics' for their child's assigned teacher"
