Looking for a SIPOC template.

I am trying to create a SIPOC (Suppliers, Inputs, Process, Outputs,
Customers). I have looked through all of the templates and have not been able
to find one for SIPOC and I do not know how to make an original chart. Please
help

I'm not aware of any such template. If you google sipoc diagram, there seems
to be quite a few examples. You can use basic Visio shapes to make such
diagrams.
