Looking for a small medical practice appointment/calendaring DB

Want to create an appointment database using Access. Database should contain
patient demographics, provide for patient scheduling, queries that lead to
making an apointment data entry which includes color coding for types of
appointment/treatment. Reports should include patient roster, mailing lists,
and calendars that print daily, weekly and monthly schedules.

write this for you? to write it yourself with our help? to download a
customized free version <g>?

I was looking for an Access "template" or program someone else had already
written (assuming someone was willing to "pay it forward"). I DO NOT want a
turnkey...WAY to expensive for the application (we have two part-time docs).
