I want to have students in a class respond by filling out and sending me an
email.

What you want to do is certainly possible, but I tell everyone who wants to customize a message form to take a look at the information at http://www.outlookcode.com/article.aspx?id=61 first, because there are some prerequisites and limitations that you need to know about before you plunge into too deeply.
