Lookup a field on a form from the table

I have a database with multiple tables & forms that all tie back into one the
main data table called "Contacts".

I want to have the fieldname "LastName" a lookup field, so that when I go
into my form (also called Contacts), I can click on the LastName dropdown box
to open a record or to know if I'm going to need to create a new one.

Can I do that? and if yes.... how. I'm really a newbie so please be gentle
as well as dumbdown your instructions!
