Lookup box

I am trying to design a database for a music lesson survey where there are a
13 different instruments at three different levels. To put each instrument
in a table with all three levels would create a lookup box with 39 records
which would be unweildy to scroll through. i would prefer to create one
lookup box for the instruments and a sub lokup box for the levels. Each
survey form returns up to 5 different choices for instrument tuition
required. I'd appreciate any help anyone can give.
