It is possible in versions since Word 97. Tools > Customize.
Make sure that the location for storing your custom toolbar is your document
rather than normal.dot or your template. If your customization involves a
macro, you want that macro stored in the document first. Unless you
deliberately put a macro in a document, it will not be there. Also be aware
that in many environments macros are blocked by macro security. You could,
instead, set it for the built in GoToTableOfContents command (as suggested
by Suzanne).
