Mail merge using Word 2003 and business contact manager

When I attempted a mail merge in Word 2003 using contacts in Business Contact
Manager the recipients full address failed to appear in the address
placeholder.

I did manage to do a mail merge from Outlook which worked apart from
randomly inserting a blank line before the City line.
Has anyone else had this problem and is there a solution?
