make a word template that adjusts daily

Hi
I have a question about word:
I have word 11 for mac. What I would like to do is have a heading load
every time that I open a new document. I know I could use a template,
but I would like the date to change.
I would like something like:

Name
12/14/05

At the top of the page

Also, how could I have this load when I start word, automaticaly?
Thanks!

That's what the CREATEDATE field is for. (see insert » field...)The
date will be the date of first creating the document from the template.
Putting it in a template is incredibly easy and leaves your options
open for the times you don't want the date there.

Don't forget all the formatting options, and remember that you may have
to select the field and update it (ctrl-click for a context menu or hit
cmd-opt-shift-u)

There are also preferences for updating all the fields in a document
before printing. And a few more tricks you will find in help, like all
the confusing keyboard shortcuts involving F9 and just about every
funny key on your keyboard, and which collide with dashboard or exposé
or something. (I left all the f9 stuff in Word and re-defined the OS X
keyboard, so long ago I forgot which)
