Make reservation and get response

Outlook2000. I'm trying to make a form build on the Appointment
form. On Sheet2 I have added several textforms, checkforms etc.
These I wish the the reciever to fill out, but when I get their
answers (the accept of the appointment) the Sheet2 is gone and I
can't see what they have typed in the textboxes etc.

Is it wrong of me to use a modified version of the Appointment
form for this kind of request? If I can use it, then how do I get
the information from the textboxes?
