Making a response form work

Hi - I have inserted a form on my web site and configured it to send
responses to my email address.

But it doesn't work !!! When the submit button is used you get "page not
found".

What am I doing wrong???

Many thanks for your response - extremely informative site. Another
question - how do I upload to the server in http and not ftp?

requires FP extensions on the server.
I recommend you look at the uploading tutorial on my tutorials page.
it covers both processes in detail.
