Making a template

Hi All,
Can someone point me in the right direction..... I'm using the
Word program that came with my computer as part of the MS Works suite of
programs.

I have a document that was sent to me. It contains some regular text
information as well as some check boxes Yes/No for certain selections. There
are also blanks for entering in a name, address, etc....

I want to scan this document into my computer and than open it in my Word
program and make a template out of it where I can type the information in or
check the check boxes, etc...

Can anyone give me some pointers, or a web site where I can go..... I looked
in the help files that come with te program but it did not explain how I can
make this file into a template. I saved it as a windows meta file when I
scaned it if that makes a difference.

Hopefully someone here can help me out on this problem.

Just click File and Save As, change the "Save as type" to Document Template,
give the template a name (no extension), and then hit save. This will save
the document as a template complete with all text and formatting. You will
then be able to use the template when opening a new document.

Hi Opus,
I got that part figured out. What I can't figure out is how
to make the document so when you open it, the cursor is at the first
location, say Name: and you can enter a name, hit the Tab key and move to
the next spot, say Address: enter that and Tab to the next spot and so
on....

Then using the tab key move from field to field on the document until
everything has been entered. Thats what I've been knocking my head against
the wall trying to figure out... Any thoughts here? :)

I don't believe you can do this with tabs. You are thinking of how you
fill out forms in Adobe or PaperPort. In Word, the tab key is used to
actually create a tab, so that won't work in templates (dot files). If
you pull up the Microsoft sample templates (under File, New), you will
see that even there documents do not let you tab from field to field.
Instead you must click on each field to enter data.

This is how you must do your fields in your template as well. The
easiest way to get those form fields in the document is to first turn
on the Forms toolbar (View, Toolbars, Forms). That will give you a
toolbar that allows you to quickly insert different types of fields
(text, checkbox, dropdown boxes, etc.).
