Manipulating the CC item/field in a custom form

I have a similar issue. My New user request form needs to
be sent to the applications group if a certain app is
required. I want to autopopulate the CC field w/a
recipient if a specific CheckBox control is checked (-1)
on the form... any help would be greatly appreciated.
