Meeting follow-up notes

I am looking for a format to capture meeting discussion and follow-up
assignments. I prefer the Excel format, but I will use Word. I look for
ease of entry formats.

I use Excel for that - what a nightmare! I just told the boss that we need
to consider using Access for this task. Excel 2003 was *not* meant to handle
the task.
