Menu Items Disappearing When Additional Global Addin Loaded

I have several addins with menus running on Windows XP and Office XP. One addin builds the menus through code and the other addin I have built the menus manually. Both addins have toolbars. In addition to the two global addins there is one additional global addins that load which does not contain menus, only code the is common across several addins

Whenever I load the second global addin the menu item of the first global addin loaded disappears. If I unload the second global addin the menu reappears

This behavior is related to a particular installation because I do not get this same behavior in Windows 2K/Office 2K or another installation of Windows XP/Office XP

If I do not set the customization to the global addin
itself and let the customization occur to the users
profile (normal.dot) my menus do not disappear.

The users have complete control over the global addins as
they sit in a directory at the root level in which they
have complete control.
