Merged data is inconsistent

Strangest thing -

I am looking at my Excel spreadsheet and my Word merged document and
the Word doc contains incorrect merged data into the fields. I have
done this hundreds of times and can't figure out what happened.

I've checked the headers, I've checked the Excel tabs, all is
accurate. Strangely, some of the fields merged correctly and some
didn't.

I am totally baffled.

In what way is the data not correct? How about some examples of what is in
the spreadsheet and what is being displayed in the merged document?
