I need an example of a training seminar template, which includes preparation,
presenter and participant issues, accommodations, and facilities in the plan.

Don't know of one but:

Last time you created a new seminar and delivered it what tasks did you do?
If you can't remember brainstorm with some other trainers. Logically link
the tasks and you have most of your template. It will also be far more
relevant to your situation than any other template could be.
