Missing Attachments

I am running Outlook 2003 with an exchange 2003 SP2. I have several clients
that are loosing attachments and inlaid images when they send messages
internally. This started with 1 or 2 clients and has grown to 20 or so
whithin a week. I have asked in several forums but so far no luck. Any help
would be greatly appreciated.

Attachment filtering? SPAM filtering? Inbox rules? Policies?

All should be default policies from outlook.
