Missing Data on Form

I created a form (FP2003) on my site to send results to an email and
everything seems to be working fine except on submission, on the confirmation
page and on the email generated, there are a couple of fields that do not
return the data that was entered. Any suggestions? Here is the link to my
page with the form.
