Missing document indicator

I've created a document from PWA and WSS (Project Server
2003 SP1) but the document icon never appears in Project
Center indicator field. The indicators for Risks and
Issues are showing without any problem. I expanded the
size of the field but the document indicator is not
hidden.

Any suggestions...thanks!

Possibly related information...On a separate test
instance, I'm getting the following message when I try to
add an Issue.
"Project Server failed to update the link between the
issue and selected tasks. Log on to Project Web Access
and link the issue to task from the Tasks, Resource
Assignments or Project Drill-down page.

Are you saying that you can link to the document if you create the link from
the View my tasks page, but not if you try to create the link from WSS?

I can create a documents, risks or issues in any manner
but receive "Project Server failed to update links in
Project Server database" when I try the Synchronize with
Project Server option if I use a database that was
migrated from 2002 to 2003 and SP1 applied. However, I've
successfully tested a vanilla install of PS 2003 and SP1
applied, importing a project and adding a document, issue
and risk. The procedures used to create the two
environments were the same except on the database side.
