Missing fields on new message form

Exchange server 2003, Outlook XP, Windows XP

One user only. New message form displays only the TO and teh message body
box on screen.

Tools>Forms>Design this form-- both the CC and Subject field show on the
form design screen, but still don;t display on the new message form.

I deleted these two fields from the form design, and then re-positioned them
onto teh form, saved the form and opened a new message-- still no CC or
SUBJECT fields are available for input. What am I missing?

Anyone have suggestions or ideas
