Ask GoDaddy to run a server health check and/or re-install the extensions for your account.

There aren't any FP extensions on that web.

http://www.jchristopherwhite.com/_vti_inf.html should return a page with
extension info...it don't.

Ask GoDaddy to run a server health check and/or re-install the extensions for your account.

FYI, reinstalling is easily done through their Control Panel. Simply
uncheck the box for "FrontPage Extensions" and they will be removed.
Recheck the box about a half hour later and they're reinstalled.

Thomas,
Just want to say thanks on your advice. I was searching and searching but
could not get the submit button to function properly. A login box would
apear when the submit button was pressed. I re-installed the extensions and
the form works propperly now.
Thanks,
