Missing Indexes and Tables menu item from Insert

I am missing the indexes and tables from the menu bar when selecting
Insert/References. I'm trying to create a table of contents and the "Indexes
and Tables" option is missing to begin to create a table. I'ven uninstalled
and reinstalled Office XP twice and it's still missing.
