Missing navigation button for new page

I think this is just a symptom of a bigger problem, but I'm hoping to find
the answer...

I added a new page to my site, and published the whole site (not just
changed pages), but the navigation button for the new page appears as the
missing image red X icon, instead.

Other symptoms I suspect are related:
1) changes to my shared borders don't appear on the rest of the pages unless
I do a "publish all pages" operation. Changed pages only, under either means
of determining change, do not update the shared borders.
2) my input forms have stopped working www.hoagiesgifted.org/feedback.htm

My site is www.hoagiesgifted.org
