missing task notes

Have a weird situation here..a certain resources task notes have been
deleted mysteriously. The PM had previously seen the notes and approved
the tasks. He checked his other resources and they all had their notes
in place.

What could be the problem???

Did the Resource themselves delete the notes in their tasks? Or, is it
possible that another resource did? Or, did something happen to the resource
themself in Project - like the resource was deleted and then re-added with
the same name?
