Moving files and Forms Problems

Hi All,

I created a very spiffy InfoPath form to populate a database. Everything
works just fine when I have the files on my computer, but when I move them to
the server this odd dialog box opens asking for various logins etc -- even
though I already re-associated the form file with the database on the server.
Can anyone help?
