MS Pub WEBBOT Error

Hi folks:

I am new to MS Publisher, I use version 2003.

I created a form so that others may contact me by filling out their
contact Information.

When they click Submit, they receive this message;

The requested URL /index_files/--WEBBOT-SELF-- was not found on this
server.

Can someone please help me fix this error?
