MS Word 2004 for Mac

Trying to create mutually exclusive check boxes on a Word form document.

Can produce regular check boxes for independent variable, but not dependent.

Can also do this as HTML option buttons, which works on browser, but not on word document.

How can I create an option box that only allows one selection box or circle selected from a group of options on the word document itself. Thanks..

I believe this will work on the Mac although anyone with Word 2008 won't be
able to use the form. As you can see it relies on a macro & Office 2008
doesn't support VBA.
