Hi

Word has a lot of built-in capabilities for forms. For starters, search the
word "forms" in Word's help.

There are several good books about Word. Here are some recommendations:
http://www.mvps.org/word/Tutorials/BookRecommendations.htm

You will need to learn some things about Word Fields and Word's Form toolbar
(View > Toolbars > Forms). Additional on line help is here:
http://www.mvps.org/word/

There's an article "How to create a form" here:
howto&article=/mac/LIBRARY/how_to_articles/officex/wd_createform.xml>

Alex:

Go to Word>Preferences>Save and enable "Save Data Only for Forms".

Look in the Help to see what that option means and how to build a form that
uses it.

Cheers
