MS-Word Form Data to MS-Access Database

Hello,
I am using MS-Word 2003 and MS-Access 2003.
I have created a form in MS-Word 2003. This form has text boxes, combo
boxes, and check boxes.
I would like to export / submit the "selected" data on the form to an
MS-Access database.
I know I have seen this before, but do not remember where.
Can someone point me to some examples or samples of my request???
