Multi-level forms possible?

Is it possible to have multi-level forms in the same way as you can do with reports?

Say I have a query producing the following data:

Department1 Name1 21/07/2004 2
Department1 Name1 23/07/2004 4
Department1 Name1 25/07/2004 3
Department1 Name2 15/07/2004 3
Department1 Name2 16/07/2004 2
Department2 Name3 11/07/2004 3
Department2 Name4 16/07/2004 6
Department2 Name4 18/07/2004 7

I would then like to display this in the following way:

Department1
Name1
21/07/2004 2
23/07/2004 4
25/07/2004 3
Name2
15/07/2004 3
16/07/2004 2
Department2
Name3
11/07/2004 3
Name4
16/07/2004 6
18/07/2004 7

In a report this is possible, but can it be done on a form?
