Multipe task options - best way to display

Hi , I have to create a form which allows the users to carry out about 12
different tasks. I had considered either creating a front end menu (but
would rather not) - then thought about creating a dsrop down menu in header
of form to allow user to select from list.

However thought I would ask you wise people which way you think would be
best. Is it possible to create a custom menu bar that would auto load when
the form loaded?
